Teacher Recommendation Request Form

TEACHER LETTER OF RECOMMENDATION REQUEST FORM


 Ask your teacher(s) in person at least two weeks in advance of your deadline.

  1. If you are using the Common Application, your teacher will upload his/her letter directly to your Common App account. (You must input his/her information at www.commonapp.org)
  2. In all other instances, have your teacher email a copy of their letter directly to your college counselor. We will then submit these teacher recs to your schools as you request them.

IMPORTANT:  Remind your reference to save these in a file on their computer, as you may need multiple copies for other scholarship and admissions applications.  Write a thank you note as a sign of appreciation!
