Teacher Recommendation Request Form
TEACHER LETTER OF RECOMMENDATION REQUEST FORM
Ask your teacher(s) in person at least two weeks in advance of your deadline.
- If you are using the Common Application, your teacher will upload his/her letter directly to your Common App account. (You must input his/her information at www.commonapp.org)
- In all other instances, have your teacher email a copy of their letter directly to your college counselor. We will then submit these teacher recs to your schools as you request them.
IMPORTANT: Remind your reference to save these in a file on their computer, as you may need multiple copies for other scholarship and admissions applications. Write a thank you note as a sign of appreciation!
